The palms-down position matters only if the wrists stay neutral.

No-Fly Zone
Avoid loads that bend the wrists back or require torso swing.
Programming
Late
It is an accessory curl that can fatigue grip and wrist stability, so it follows heavier pulling and arm work.
Model — Double
Fallback model — Density
Keep palms down, wrists neutral, and elbows quiet; add reps before moving up one dumbbell step.
Pull
If it hurts
Pain is information, not a challenge to push through. Stop the set if pain is sharp, worsening, or changes how you move. These are exercise-selection options, not medical diagnosis or rehabilitation advice.
Work down this list in order. Stop at the first change that removes the pain.
Step 1
Reduce load, use alternating reps, or support the upper arms against a pad.
Step 2
Use a three-second lower or a one-second midrange pause.
Step 3
Cable reverse curl or direct wrist extension.
Substitutions
Cable reverse curl or direct wrist extension.
The Evidence
This changes elbow-flexor and wrist-stabilizer demands compared with a neutral or supinated curl. It is more than a cosmetic grip change, but it is not isolated wrist extension.
